Information and availability for Provider Offices and Shingles Vaccine

One of the newer preventive vaccines approved for TRICARE is the shingles vaccine (Zostavax).
TRICARE beneficiaries age 60-65 may contact your office to obtain an appointment and/or confirm that you can provide the shingles vaccine to them.
 
Humana-Military HealthCare Services now has a specialty pharmacy contract with CuraScript who has agreed to supply the shingles vaccine (when available) to provider offices. Having the specialty pharmacy provide your office (by order) with the shingles vaccine eliminates your need to refer out for the vaccine. There are strict handling guidelines for the shingles vaccine and you should verify with CuraScript that your office can receive and store the vaccine properly.  CuraScript also bills directly to TRICARE for the order, eliminating the need for the provider office to incur the cost of the vaccine and billing to PGBA for the drug.

This fax order form is downloadable to print from our website for your office. You will need to fill out the prescription order form and fax to CuraScript for your shingles vaccine request.  Please be sure to contact CuraScript at 1-888-773-7376 to confirm the vaccine is in stock. If it is not, CuraScript will take your information and ensure you are contacted when the vaccine becomes available.
 
On behalf of the TRICARE providers in the SOUTH Region, Humana-Military HealthCare Services is informing eligible beneficiaries of the shingles vaccine shortage and requesting that they contact their provider to see if the office has the vaccine or will be receiving the vaccine to dispense to TRICARE beneficiaries. We are also asking that the beneficiaries make an appointment with the provider in advance so that you are not ordering the vaccine blindly. Please be reminded the shelf life of the vaccine is very short and requires special handling and dispensing by the provider in a very limited timeframe.
